NYC DOE LEARNING ENVIRONMENT SURVEY

For the fourth year, New York City's Department of Education is conducting an annual survey of parents, students, & teachers -- to learn how schools can better serve our students. You are invited to take the survey to help the DOE obtain a better picture of how our school is performing.

Please remember how important it is that every family completes their survey. Survey results determine 10% of our school's yearly grade, and our grade can affect our funding. All the instructions you need to complete your survey are inside your green envelope.

COMPLETE IT ONLINE: It is fast and easy. Just go to http://schools.nyc.gov/surveys and use the code provided in the survey envelope mailed to your home by the DoE. Or you can fill in the paper survey, and pop it in the mail in the postage-paid envelope provided. (Do not return the survey to the school.)

IF YOU LOST OR TOSSED YOUR SURVEY ENVELOPE: Please call the survey hotline and they will give you your code number so you can complete the survey online.

The Survey Hotline 1-800-690-8603 will be open 24 hours a day, 7 days a week. If PARENTS or TEACHERS have lost their survey ID code, they should call the Survey Hotline to request a replacement serial ID. For additional information parents can call 311.

Survey responses are due by Friday, April 23rd!

To see the results of last year's survey or for more information, go to http://schools.nyc.gov/surveys.

Surveys can be completed online in the following languages: LEARNING ENVIRONMENT SURVEY

For more information contact the survey team at at surveys@schools.nyc.gov
or (212)374-6646.
